Accessing Your Records
How patients may request access to personal and clinical information.
You have the right to ask for a copy of the information we hold about you. This is called a subject access request. There is no charge in most cases.

How to make a request
Write to our Data Protection Lead with your name, date of birth, contact details and, if you can, the date and location of the incident. We will ask you to verify your identity before releasing anything.
We will respond within one calendar month. That period can be extended by up to two further months for complex requests, and we will tell you if that happens.
Very occasionally information is withheld — for example where release would identify a third party who has not consented, or would be likely to cause serious harm. If we withhold anything, we will tell you that we have done so.
